Bark at the Moon Animal Rescue is a volunteer-based, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected, and abused animals. Bark at the Moon Animal Rescue is completely funded by the support of the community through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive complete medical care, which includes de-worming, vaccinations, prevention, and spaying/neutering (if of age)  prior to their adoption.

This questionnaire is designed to help Bark at the Moon Animal Rescue and yourself determine that you are a fit for adopting from BATM, and identify what you're looking for in your future pet! Then we do all of the hard work in finding you a perfect match. All adopting parties are required to complete this form prior to adoption. Bark at the Moon reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!
